For application to the Phenotyping course you need to fully complete this form. Incomplete forms will not be considered. As soon as it has been processed we will send you notification about whether you have been accepted. Regrets of a confirmed admission to the course must reach us at Office of Education at Comparative Medicine NOT LATER THEN November 2, 2015. Cancellation of your course place after November 2, 2015 will be charged an administrative fee of 4000 SEK to your Department/Institution.
